Our physicians and staff know how important it is that your phone calls be returned promptly. To accommodate your expectations, while providing timely care to those in the office for scheduled appointments, we have set up the following system:
There is always a physician from our practice on call to assist you, 24/7. After hours, your call to our main number (312-726-7272) will be routed to our answering service and you may receive a response from one of our providers or from a covering physician.
To continue your medications, we ask that you plan ahead for prescription refills. We will make every effort to assure that you are aware of restrictions and limitations on our medications at the time they are prescribed. Of course, you may always call our office with questions. Your prescription will be sent to the pharmacy of your choice once your prescribing physician approves it.
Rest assured, our physicians make every effort to review test results as promptly as possible. Generally, lab results take up to five - seven days for review. However, complex lab results may take up to two weeks. You will be contacted with normal or abnormal results. If you have questions or concerns about your test or the treatment plan your physician has recommended, please discuss it during your next visit.
At Chicago Women's Health, like most other physician practices, we have been increasingly challenged by the many medical forms which we are asked to complete each day on behalf of our patients. While we have provided this service at no charge for decades, we now find that we cannot continue to divert our limited resources in this manner without harming our ability to provide attentive, personal care to our patients.
In consideration of the time required by our physicians and staff to complete patient forms, we have implemented a $35 per document Administrative Service Fee, effective Jan. 1, 2005. This fee applies to all forms, excluding those that essentially require only a physician signature, like a post-operative disability or short-term disability form for maternity, and specialty referral forms. It also excludes forms that are completed during an office visit in which the information needed for the form is collected. Should you need our practice to complete medical forms on your behalf, please:
While we make every effort to simplify our billing statements and assure their accuracy, we know that questions arise. On each bill you receive for care provided by Chicago Women's Health there is a phone number to contact our Billing Dept. Don't hesitate to call if we may be of assistance.
All offices require that co-payments and outstanding balances be paid at the time of service. However, if you find yourself in need of financial assistance, our administrative staff will be happy to help you make the payment arrangements you may need.
